**THIS CLASS IS HELD AT 2420 E. PIKES PEAK AVE. (MEMORIAL ADMINISTRATIVE CENTER) IN ROOM 1024.  It will be two 10 hour days, February 21 and 28, 2016
YOU MUST BE A COLORADO STATE CERTIFIED EMT TO TAKE THIS COURSE. A copy of your State certification must be provided to our office.

You must also submit proof of receiving the Hepatitis B Vaccine OR a signed Hepatitis B Vaccine Declination Form (form is available in our office).  Curent Healthcare Provider CPR and flu vaccination for 2015-16 are also required.  Please bring COPIES of all documentation on the first day.
