Continuing Education Credits, Registration, & Fees 
     Penrose-St Francis EMS Education is providing the continuing education credits for this conference. Student must attend the lecture or skills station to receive credit for said lecture or station. Enrollment for the Plains to Peaks EMS/Trauma Conference is open to all interested parties. Expected attendees include but are not limited to: First Responder, EMT-Basic, EMT-Intermediate, EMT-Paramedic. Course cost is $20 each day including breakfast, breaks, and lunch. Registrations should reach the Regional office by April 23, 2010 to avoid late charges and assure seating.

Saturday, May 1, 7:00 PM
Come join with fellow emergency responders as we recognize and applaud the accomplishments of these deserving health care providers from across the Plains to Peaks Region. Dinner will be served at 7:00 PM followed by an the awards presentations and an evening of entertainment.

Registration for the Awards Program and Entertainment is $20 and can be included on your Conference registration. Please register early so we can adequately plan for seating and food. This will be a special event and you are encouraged to put on your best jeans and come enjoy a fun evening as we recognize these deserving award winners. Who knows, we might even have a wedding!

GENERAL CRITERIA: Nominees for an award must be affiliated with an Emergency Response agency, Trauma Facility, or Emergency Dispatch agency in the Plains to Peaks Region. The Region includes the Counties of Cheyenne, El Paso, Kit Carson, Lincoln, and Teller. All award categories except the Volunteer and Career Lifetime Achievement awards should reflect a significant contribution in the delivery of EMS during the past year. This may include but not be limited to a single event or an accumulation of events that demonstrate the dedication and commitment of the individual or agency during the past year. The Lifetime Achievement awards should reflect a significant contribution by the individual over a period of years that demonstrates an accumulation of events or dedication that goes beyond what would normally be expected. Please attach additional pages to the nomination form as necessary. Letters of support are encouraged but not required.

FIRST RESPONDER, EMT BASIC, EMT INTERMEDIATE, EMT PARAMEDIC: A nominee for this award must be a certified responder at the appropriate level. A Volunteer nominee is considered a responder who may or may not receive a stipend for their work but for whom this is not a full time job. A Career nominee is a responder employed full time at the position for which they are nominated. The nominee should have demonstrated work of a significant nature during a single event or an accumulation of events during the past year that goes beyond what would normally be expected.

MEDICAL DIRECTOR: A nominee for this award must be currently affiliated with an agency providing medical oversight to emergency response personnel in the Plains to Peaks Region. The nominee should have demonstrated significant involvement with the agency that has helped to improve the delivery of emergency medical services in the past year.

DISPATCHER: A nominee for this award must have been employed by the dispatch agency in the Plains to Peaks Region at the time of a significant event or events that required the individual to go beyond what would normally be expected.

ER PHYSICIAN: A nominee for this award must have been employed at a designated trauma center in the Plains to Peaks Region at the time of a significant event or events that required the individual to go beyond what would normally be expected.

ER or FLIGHT NURSE: A nominee for this award must have been employed at a designated trauma center or air ambulance service in the Plains to Peaks Region at the time of a significant event or events that required the individual to go beyond what would normally be expected.

INSTRUCTOR: A nominee for this award must be an instructor in an EMS related subject such as but not limited to emergency medical dispatch, pre-hospital care, or clinical care. The nominee should have demonstrated significant involvement in education that has helped to improve the delivery of emergency medical services in the past year.

EMS AGENCY: A nominee for the award must be an agency actively involved in the delivery of pre-hospital care within the Plains to Peaks Region. The nominee should have demonstrated delivery of services in the past year beyond what would normally be expected.

VOLUNTEER LIFETIME ACHIEVEMENT: A nominee for the award must have volunteered with a Regional agency for at least the past year. The cumulative contribution by the nominee should be such that it stands out as an example for other volunteers to emulate.

CAREER LIFETIME ACHIEVEMENT: A nominee for the award must have been employed full time with a Regional agency for at least the past year. The cumulative contribution by the nominee should be such that it stands out as an example for others in the same career.

NOMINATION SUBMISSION DATE: The award nomination form will be available at any time on the Regional website, www.plainstopeaks.org or by request from the Regional office. Nominations must be received by the Regional office by April 1st of the award year.

AWARD NOTIFICATION DATE: Award nominees will be notified by the Regional office by April 15th of the award year.

AWARD PRESENTATION DATE: Awards will be presented during the annual Plains to Peaks EMS & Trauma Conference. The Conference is held near the end of April each year.

DATES IN GENERAL: Any changes to the above dates will be made public and every effort will be made by the Regional office to alert interested parties in the Region to such changes in a timely manner.

AWARD SELECTION COMMITTEE: The Plains to Peaks Regional Council is made up of stakeholders from each County in the Region. The Council will select a committee from among themselves, other stakeholders, or such combination as the Council deems appropriate.

GENERAL: The Regional Council views this program as a way to recognize and reward the efforts of exemplary personnel across the Region. Every effort will be made to make the process fair and equitable. Any concerns, questions, or suggestions should be directed to the Regional office.
